Our client is seeking an experienced and clinically trained Complex Case Manager to support clients and their families through high-quality, trauma-informed case management. In this role, you will manage a caseload of clients with complex psychosocial and health needs, providing clinical expertise, coordinated support, and guidance in navigating systems and external services.

The successful applicant will play a key role in enhancing client wellbeing, by applying the clients Wellbeing Model, and ensuring consistent, high-standard case management practices across the Client Support Program.

The Duties:

  • Provide specialised, trauma-informed clinical case management to clients with complex needs.
  • Conduct intake assessments, risk assessments, and case reviews.
  • Act as a primary point of contact for allocated clients, coordinating access to mental health, medical and external support services.
  • Escalate high-risk client issues and work closely with Senior Clinicians and Assistant Directors.
  • Build strong relationships across all business areas, including ex-service organisations, and community providers and Federal Government Agencies.
  • Prepare high-level documentation, briefs and client summaries.
  • Support colleagues with clinical advice, coaching and guidance for managing challenging presentations.
  • Represent the program through group presentations and stakeholder engagement as required.

Essential Requirements:

  • Tertiary qualifications (Bachelor or above) in Nursing, Psychology, Occupational Therapy or Social Work.
  • Demonstrated experience supporting people with complex and/or co-morbid needs.
  • Strong understanding of disability impacts and capacity-building support models.
  • Excellent communication and stakeholder engagement skills.
  • Experience in assessment, triage and working within structured case management frameworks (including telephone-based work).
