Join our Transition Services team as a Social Worker today

As the Social Worker, you will support patients who are on the Care Awaiting Placement South (CAPS) service as they transition from the acute or subacute service to residential aged care services. You will be involved in triage and assessment of patients, coordination of patient services and continual monitoring of services provided by the CAPS providers.

About the role:

As the Social Worker - Care Awaiting Placement South, duties for the role will include:

  • Supporting triaging referrals
  • Providing consultancy service to other staff and agencies re provision of health services for older people on a residential caged care pathway
  • Working collaboratively within a multidisciplinary team to continually improve, implement and evaluate a holistic CAPS program which is responsive to identified community needs
  • Supporting care co-ordination for clients on the SALHN CAPS Program through regular contact with service providers and the acute and subacute sector

About you:

  • Demonstrated skills in case management/care coordination and the ability to make complex clinical decisions including sourcing and coordinating services to meet clinical and community care needs
  • Demonstrated experience working within a multi-disciplinary team with contribution to the development and attainment of team goals
  • Demonstrated sound clinical experience with older people in acute and/or community settings
  • A degree in Social Work (eligibility for membership of the AASW)
  • Proven experience in delivering high quality and safe care consistent with the National Safety and Quality Health Care Service Standards
