**We are seeking a dedicated and skilled Social Worker to join our Rehabilitation, Aged Care and Palliative Care Team.**

The Social Worker participates as a member of the inter-disciplinary team to effectively plan and provide therapeutic interventions based on patient goals. This role works as a part of a 7 day roster working part of the Complex And Restorative Team (CARE) which includes working in the Geriatric Emergency Department In-Reach (GEDI) and across the service streams as required.

**About the role**:
Reporting to the Advanced Nurse Unit Manager CARE Services and Director of Social Work, As the Social Worker you will be involved in providing a comprehensive range of social work services to patients requiring specialist geriatric evaluation, assessment and care within the designated service.

Duties for the role will include:

- Collaborating with other team members and community agencies
- Documenting assessment findings and interventions in accordance with service standards
- Linking clients and carers with relevant community resources and services
- Collaborating with other team members to initiate, develop, implement, and evaluate therapy/activity groups
- Providing a consultative service to other team members, disciplines and agencies

**About you**:

- Ability to critically analyse or evaluate issues related to clinical care in social work in a manner which can be applied to quality activities, service evaluation or research
- High level interpersonal and negotiating skills and the ability to communicate with clients and staff at various levels, both orally and in writing
- Experience as a social worker in a health care setting
- A degree or equivalent in Social Work, entitling membership of the Australian Association of Social Workers
- Proven experience in delivering high quality and safe care consistent with the National Safety and Quality Health Care Service Standards
