We are seeking a dedicated and passionate Occupational Therapist to join our Geriatric Outreach Service, which delivers care to patients in Residential Aged Care Facilities (RACFs) to support the transition process and care in RACFs to avoid preventable admissions to hospital.

About the role:

Reporting to the Advanced Nurse Unit Manager, CARE and the Director Occupational Therapy, this role involves working closely with the Senior Occupational Therapist, Geriatricians and Nursing team in the Geriatric Outreach Service. The role supports patients with memory loss and Behaviour and Psychological Symptoms of Dementia (BPSD) in their residential care environment to develop and action patient centred management plans to support patient's well-being, and support integration of care between SALHN and the facility - by providing direct patient care and educating and supporting RCF staff to care for patients with memory loss and BPSD in line with the patient's documented management plan.

Duties for the role will include:

• Assessing patients' occupational abilities, problems and needs using a variety of formal and informal assessments,
• Participating in interdisciplinary clinical and planning activities to enhance the overall service provided to patients,
• Devise patient programs and behavioural support plans and supervise staff in their implementation of these,
• Undertaking Occupational Therapy projects, especially those related to the Geriatric Outreach Service.
• Assisting in the development of strategies, procedures, protocols for provision of Occupational Therapy interventions

About you:

• A degree or other qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia,
• Experience as a clinician in the assessment and provision of high-quality Occupational Therapy services to clients with a variety of conditions requiring management of issues associated with cognitive impairment and/or changed behaviours and behavioural support planning,
• Ability to critically analyse or evaluate issues related to Occupational Therapy care in a manner which can be applied to quality activities, service evaluation, professional education or research with a commitment to ongoing professional development,
• Experience in establishing and maintaining high standards of Occupational Therapy intervention,
