About The Position
The
Aftercare Case Worker
is a new position under the national Culture Care Connect (CCC) program that brings together key streams of suicide prevention planning, coordination and activity across the Perth network including raising awareness, early intervention, crisis management and aftercare services. The position is integral to the development of a suicide prevention and response model of care – collectively, working to improve Aboriginal mental health and suicide prevention outcomes by providing culturally appropriate aftercare service and support.
This will be a Maximum term Full-Time contract which expires on 30 June 2026, but with possibility of extension.
About The Organisation
The Derbarl Yerrigan Health Service Aboriginal Corporation (DYHS) was established in 1974 and is the largest Aboriginal community-controlled health service provider in Western Australia, providing care to over 20,000 Aboriginal patients across four clinics in East Perth, Mirrabooka, Maddington and Midland.
Our staff enable the delivery of culturally responsive primary health services, focusing on prevention, early intervention, comprehensive care and care coordination including chronic disease management.

Responsibilities
- Contribute to a localised service delivery model for aftercare services through a co-design approach with local Community, using the National Aboriginal Community Controlled Health Organisation (NACCHO) National Model of Care, and other resources, as a framework.
- Provide aftercare services to Aboriginal people who have been referred following a suicide attempt or suicidal crisis in line with agreed service delivery model.
- Build and maintain strong referral pathways, including but not limited to the following services: acute care services, primary care services, schools, police, funeral services, Alcohol and Other Drug Services, mainstream and Aboriginal and Torres Strait Islander-led mental health services.
- Participate in the Aftercare Services Community of Practice, established to support ACCHOs in setting up and maintaining Aftercare Services, and facilitate sharing of best practice and expertise.
- Maintain client confidentiality.
- Work collaboratively with and across Derbarl services to optimise program outcomes.
SELECTION CRITERIA
Essential
- Aboriginal and/or Torres Strait Islander pursuant to Section 50D of the Equal Opportunity Act.
- Minimum Certificate IV in Mental Health or Aboriginal and/or Torres Strait Islander Primary Health Care or equivalent training and/or minimum two years' experience within the field of mental health or suicide prevention.
- Excellent communication and engagement skills
- Demonstrated understanding of relevant legislation and policy frameworks within the Suicide prevention/aftercare areas
- Experience in working with Aboriginal patients and communities.
- A working understanding of client confidentiality and privacy as well as demonstrated understanding of cultural safety and its application in service delivery.
- Demonstrated ability to work under pressure, organise and plan to effectively manage a complex working environment including ability to perform a range of interventions with limited supervision and exercise initiative while using discretion and sound judgment.
- Demonstrated sound professional judgement and ability to work collaboratively within a team as well as with external services.
- Good level of computer literacy.

- Satisfactory National Police Criminal History check (less than 6 months old)
- Current Working with Children Clearance
- Current Driver's Licence.
- Successful Pre-Employment Health Assessment
