Position Summary:

We are seeking a qualified and compassionate Social Worker / Triage Officer to support the triage and care of our most at-risk veterans. This role is critical in ensuring sensitive and appropriate handling of veterans' mental health needs as they navigate support services. The successful candidate will assess needs, provide frontline support, and make informed recommendations regarding referrals for further mental health assistance from the Department of Veterans' Affairs (DVA).

Note: This position is a completely remote, work from home role.

Key Responsibilities:
  • Conduct initial assessments and triage of veterans presenting with complex needs, particularly mental health concerns.
  • Identify at-risk individuals and ensure timely escalation to appropriate support services.
  • Liaise with internal teams and external agencies, including DVA, to coordinate care.
  • Maintain accurate records and case notes in line with privacy and confidentiality requirements.
  • Monitor and review cases, making recommendations on when further mental health interventions may be needed.
  • Support veterans in navigating the system and accessing appropriate services.
Essential Criteria:
  • Tertiary qualifications in Social Work or a related field.
  • Experience working in mental health, trauma-informed care, or with vulnerable populations.
  • Strong understanding of veteran issues and DVA processes (or willingness to learn).
  • Excellent interpersonal, assessment, and documentation skills.
Desirable:
  • Experience in a triage or intake role. (4+ years)
  • Previous work with veterans or military-affiliated clients.
