The Practice

We are a long standing private practice located in Bourke St Surry Hills. Our team is committed to providing excellent healthcare, well being and illness prevention for all.

About The Role

We are looking for an experienced and self-motivated RN for permanent, part-time or casual role at a competitive salary for 2-5 days per week.

You will be providing comprehensive nursing care with a focus on chronic disease management, you will:

  • assist our GPs in the preparation, delivery and review of care plans, team care arrangements and health assessments
  • health coaching to support patients with achievement of their health goals
  • liaise with allied health professionals to co-ordinate patient care
  • Utilise clinical systems to ensure patient recalls are in place and followed up
  • Ordering vaccines and responsible for cold chain management
  • Ordering clinical supplies
  • Assisting doctors with procedures
  • Working closely with the admin team, doctors and allied health to provide best care to our patients.

About You

To be considered for this, role you will:

  • Have current AHPRA registration
  • Have a minimum of 1 years' post-registration experience
  • Ideally, have demonstrated experience in the provision of chronic disease management including experience in health coaching and behaviour change
  • We will provide training for chronic disease management if you have not had prior experience in this area
  • A high level of communication skills to work effectively with people from different backgrounds
  • Be computer literate
  • Basic computer skills including email and internet use are sufficient for this role
  • Have an understanding and dedication to patient care and service delivery standards
  • Experience working in a fast paced, varied environment
