About the Role:

Conscious Care Support is seeking dedicated Outreach Mental Health Support Workers to provide psychosocial, emotional, and practical support to individuals with chronic psychosocial disabilities. In this role, you'll:

Work independently in clients' homes and the community

Support individuals in identifying and achieving their goals

Collaborate with care teams for high-quality, client-centred support.

This is an outreach role, meaning you'll work autonomously while still being part of a supportive, tight-knit team.

What You Need:

Cert IV in Mental Health (minimum)

Strong case note writing skills (concise & articulate)

In-depth understanding of mental health and psychosocial disability

A genuine passion for empowering others

A driver's license & comprehensively insured vehicle

A mobile phone for communication with clients and stakeholders

Additional Requirements:

NDIS Workers Screening Check

Police Check (from within the last 12 months)

Working with Children Check (WWCC)
