About You
We are seeking dedicated Age Care Assessors to join our multidisciplinary Health team working across Metro Melbourne's Southern & Eastern suburbs.
This role will see you deliver aged care assessment services to older Australians to enable them to remain living independently at home.
Preferred candidates will have;
- Current experience conducting home support or comprehensive aged care assessments (due to the casual nature of the role and the training requirement required to undertake the role, we are only seeking applications from those already conducting assessments in either a home support or comprehensive capacity.)
- Ability to work independently and as a team.
- High level of computer literacy skills
- Ability to follow guidelines relevant to the position as well as demonstrated problem-solving capacity.
- Current Australian driver's license with own reliable, fully insured vehicle
- Valid National Criminal History Check and Working with Children Check - essential
What You'll Do
You'll play a major role in conducting assessments in the community, working both independently and autonomously, but be part of a broader team. Your duties will include, but are not limited to:
- Completing face-to-face assessments to determine older Australians' eligibility for support within My Aged Care
- Reviewing previously assessed clients.
- Undertaking client-centred support planning using a wellness and reablement approach.
- Liaising with referral sources and community support organisations to assist our clients to obtain the home supports they require.
- Ensuring accurate and timely completion of assessments and documentation, utilising the My Aged Care online portal.
