We are seeking a Clinical Care Partner to join our Support at Home team and make a real difference in the lives of our clients.

About the role

As a Clinical Care Partner, you will deliver high-quality, person-centred, culturally safe clinical care management services to older people living at home. Working in alignment with the Support at Home program and our clinical governance framework, you will play a vital role in ensuring clients maintain independence, wellbeing, dignity, and safety in ways that reflect their culture, identity, and preferences.

You will:

  • Undertake comprehensive clinical assessments, care planning, and service coordination.
  • Provide evidence-based clinical risk assessment, education, and guidance for clients with complex needs and chronic health conditions.
  • Offer clinical oversight and support to non-clinical Care Partners, ensuring early identification and management of clinical risks and deterioration.
  • Ensure services are delivered in line with the Strengthened Aged Care Quality Standards.

About you

You are a dedicated and compassionate health professional who thrives on building meaningful connections with clients and colleagues. With a strong clinical background, you are confident in providing oversight and coaching, while ensuring the highest standards of care.

You will bring:

  • Current registration with AHPRA as a Registered Nurse (or equivalent clinical qualification).
  • Experience in community care, aged care, or chronic disease management.
  • Strong clinical assessment and care planning skills.
  • The ability to work collaboratively across multidisciplinary teams.
  • A commitment to person-centred, culturally safe care.
