About the Role
As a Health Coach you will coordinate referrals and recruitment of clients to the Health Management Coach Program (HMC) and ensure that you deliver high quality patient-centered care. You will coach clients via telehealth using a self-management approach to achieve optimal management of individual risk factors in relation to cardiac disease and/or diabetes.
As a Dietitian, you will provide targeted dietary assessment, intervention, lifestyle coaching and clinical management of dietary related conditions and health behaviors.
Provision of telephone based coaching to promote lifestyle modification and optimal self-management of health and wellbeing; in accordance with the risk factor targets set by the National Heart Foundation of Australia and Diabetes Australia.
Manage and respond to referrals in a timely manner and recruit clients suitable for the Health Management Coach Program.
Undertake holistic comprehensive assessments, collaborative care planning and monitoring achievement of client goals using a self-management approach.
Direct clinical provision of high quality and evidence-based dietetics specific assessment, intervention, education and management
Monitor health outcomes and plan for the transition and exit from the program
Classifications for this Grade 1 Dietitian position will range from classification range AI1 - AI7 ($64, $92,123.20 per annum).
Your Contribution
Degree/Masters in Nutrition and Dietetics from an accredited institution
Eligibility for Accredited Practicing Dietitian status with the Dietitians Association of Australia
Experience working with clients living with chronic diseases
Demonstration of knowledge/experience of the provision of nutrition assessments, diet therapy and evaluation of the achievement of dietary goals.
Effective communication skills (adapting communication styles to suit clients, families and/or carers, other service providers and colleagues).
Current National criminal history check, or willing to obtain

1.LOCAL WORK ENVIRONMENT
This position is part of the Health Independence Program (HIP), which is comprised of four streams: Pain Management, Disability Services, Complex Care/Chronic Disease and Access, Rehab & Clinics. The intent of all HIP services is to deliver improved outcomes for clients and to support hospital demand management and flow, by delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centered care planning.
This position will be part of the HIP Complex Care and Chronic Disease (CC&CD) team. The key aim of the CC&CD service is to improve health outcomes for people who either frequently present, or who are at risk of presenting to hospital, because of complex medical and or social issues. Clinical service delivery is undertaken in the clients' own home, their local community and via telephone.
This position is based in the first instance at the Fitzroy site. This position may be required to work at any of the HIP hub sites or programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training.
HIP at SVHM is delivered in partnership with North Richmond Community Health (NRCH).
2.POSITION PURPOSE
As a Health Management Coach, clinical service delivery includes the provision of telephone based assessment, identification of intervention requirements based on risk factors and lifestyle modification to promote optimal self-management of health and wellbeing; in accordance with the risk factor targets set by the National Heart Foundation of Australia and Diabetes Australia.
Clinical tasks include completing comprehensive health assessments, regular follow up of clients including education and the use of self-management approaches to achieve optimal management of individual risk factors in relation to cardiac disease and/or diabetes. Liaison with other health care professionals is essential to the coordination of holistic and person-centred care and focuses on linking clients into hospital/community services needed to address ongoing requirements for improving health and wellbeing.
As a Dietitian, you will provide targeted dietary assessment, intervention, lifestyle coaching and clinical management of dietary related conditions and health behaviours.
5. POSITION DUTIES
- Telephone based assessment, identification of intervention requirements based on risk factors and lifestyle modification to promote optimal self-management of health and wellbeing; in accordance with the risk factor targets set by the National Heart Foundation of Australia and Diabetes Australia.
- Identification of appropriate referrals, responding to referrals in a timely manner and recruiting clients suitable for the Health Management Coaching Program.
- Undertake holistic comprehensive assessments in collaboration with the client using motivational interviewing and self-management approaches.
- Direct clinical provision of high quality and evidence-based dietetics specific assessment, intervention, education and management via phone
- Provision of clinical intervention such as diet therapy and evaluation of the achievement of diet and lifestyle improvements.
- Collaborative care planning with clients, carers and/or families in the context of their health, functional, psychosocial, cultural, environmental and economic needs.
- Facilitation of client generated goals, that are realistic, time limited and based on addressing health and wellbeing needs.
- Monitor health outcomes and plan for the transition and exit from the program, ensuring appropriate long term community supports are in place on discharge.
- Referral to, coordination of and liaison with the clients medical specialist, general practitioner and other health care providers relevant to the long term health and wellbeing needs of the client
- Recognise and respond to risk through timely escalation to Clinical Coordinator and Manager, participating in the development of and implementing risk mitigation plans.
- Participate in multidisciplinary case review meetings
- Actively participate in clinical supervision including preparing for meetings, being open to learning and improving clinical practice skills and implementing agreed actions.
- Communicate effectively and professionally with a wide range of team members, HIP colleagues, key stakeholders and external service providers.
- Participate in quality initiatives, team processes and service development activities for the Health Coach Service.
- Perform within the HIP guidelines and specific service guidelines, ensuring that care is provided in accordance with the HIP model of care.
- Ensuring all documentation, client records and activity recording are completed and managed in a timely manner consistent with SVHM and HIP policies and procedures, legislative and regulatory requirements.
- Demonstrate accountability to HIP through professional communication, active participation, information sharing, evaluation, statistical / data collection and analysis.
- Meet Activity Based Funding target requirements.
6. INCUMBENT OBLIGATIONS

8.1 ESSENTIAL REGISTRATION, LICENSE OR QUALIFICATION REQUIREMENTS
- Degree/Masters in Nutrition and Dietetics from an accredited institution
- Eligibility for Accredited Practicing Dietitian status with the Dietitians Association of Australia
- Experience working with clients living with chronic diseases
- Demonstration of knowledge/experience of the provision of nutrition assessments, diet therapy and evaluation of the achievement of dietary goals.
- Experience/Knowledge in the assessment and management of risk factors including smoking cessation, diet modification and physical activity.
- Ability to facilitate client centred goals and structure interventions for goal attainment based on a comprehensive assessment and using telephone coaching methods.
- Ability to perform comprehensive assessments and develop collaborative plans that address physical, psychosocial and functional aspects of the client.
- Effective communication skills (including verbal, written and skills in adapting communication styles to suit clients, families and/or carers, other service providers and colleagues).
- Able to work collaboratively with a wide range of health professionals and social care services to facilitate the achievement of health and wellbeing outcomes.
