Job Description:
We are seeking a dedicated and highly organised Cert IV(Aged care or individual support or Disability support) or Enrolled Nurse (EN) to join our growing team in Penrith. This dual role combines your skills as care coordination and administrative responsibilities, providing vital support to our aged care clients in the community. You will utilize your nursing knowledge to assist clients with their healthcare needs while ensuring their care plans are effectively managed and coordinated.
Key Responsibilities:
- Community Aged Care Support (Clinical):
- Provide clinical support and advice to clients and their families in the community, under the direction of a Registered Nurse (RN).
- Conduct health assessments and monitor client wellbeing.
- Administer medications and wound care as prescribed and within the scope of EN practice(if an EN).
- Educate clients and their families on health management strategies.
- Collaborate with other healthcare professionals to ensure coordinated care.
- Maintain accurate and up-to-date client records.
- Care Coordination (Administrative):
- Coordinate care services for clients, including scheduling appointments, arranging referrals, and liaising with service providers.
- Develop and maintain care plans in collaboration with the RN and other members of the care team.
- Monitor the effectiveness of care plans and make adjustments as needed.
- Act as a point of contact for clients, families, and service providers.
- Manage client documentation and ensure compliance with relevant regulations and policies.
- Handle client inquiries and resolve issues promptly and professionally.
- Support with auditing client files and prepare reports.
- General administrative tasks as required.
Qualifications & Experience:
- Cert IV or Current Enrolled Nurse (EN) registration with AHPRA.
- Previous experience working in aged care, either in a residential or community setting, is highly desirable.
- Excellent organisational, time management, and problem-solving skills.
- Strong communication and interpersonal skills, with the ability to build rapport with clients, families, and colleagues.
- Proficiency in computer skills, including Microsoft Office Suite and electronic care management systems.
- Demonstrated attention to detail and accuracy.
- Ability to work independently and as part of a team.
- Current driver's license and reliable vehicle.
- National Police Check (or willingness to obtain).
