Dependable Care DRPS provides disability supports across Victoria, Queensland and Tasmania. Dependable Care DRPS is a registered NDIS, TAC and Workcover provider. Dependable Care DRPS provide Support Coordination and Psychosocial Recovery Coaching.
We are committed to providing an individualised, meaningful and person centered service. We specialise in providing services to participants with psychosocial disabilities and specifically people that experience homelessness, lack of line of support and in need of a support network to help them navigate through tough times.
Dependable Care DRPS is a family run business. Within our offices and service delivery we take a family approach with everyone.
The Role
As a Support Coordinator you will be responsible for supporting Participants to activate, interpret and implement their NDIS Plans.
As a Dependable Care DRPS Support Coordinator you will be working with clients who have Mental Health or Psychosocial Disabilities. You will coordinate service providers and supports, liaise with government agencies, obtain quotes for services, organise service agreements and monitor participants plans. You will also provide regular Support Coordination reports to the NDIA for each Participant you support and your team leader.
Tasks & responsibilities
- Liaising and building rapport with key stakeholders such as: Other NDIS Providers, community venues, community partners and allied health care teams.
- Monitor plan outcomes and expenditure to ensure the services remain within the plan budget for participants.
- Independently manage a varied caseload of participants in the community ensuring quality of care and meeting client objectives and requirements.
- Conducting initial assessments
- Researching and referring participants to relevant services when required with a focus on Recovery.
- Ensure participants are working towards achieving their goals and being able to clearly document progress in reports
- Working towards KPI's set around billable hours, reports and meeting deadlines
