Ndis Support Coordinator

**The role**:
The role of the Support Coordinator is to support National Disability Insurance Scheme (NDIS) Participants to achieve their NDIS plan goals, by coordinating informal supports, mainstream supports and funded supports.
**Your key responsibilities will include**:
- Assist NDIS Participants to identify and plan strategies to achieve their personal goals within a person-centred framework
- Support clients to be implement their NDIS plan to maximise the value for money they receive from their supports
- Support Participants in facilitating their independence by enabling empowerment and choice, and the promotion of personal resilience and social inclusion. Community engagement and education
- Prepare plan review reports and other requested reports
- Build and maintain relationships with internal and external stakeholders
- Actively participate and engage in team meetings and initiatives
- Assist with problem solving and address any emergent issues
- Identify any communication or workflow issues and makes suggestions for improvement Legislative and Policy Compliance
- Operates within relevant legislation, and NDIA policies and guidelines
**To be successful in this challenging role, you must have the following**:
- Tertiary qualifications in Social Work, Psychology, Occupational Therapist, or Minimum Certificate IV in Disability Services or similar and/or equivalent knowledge and experience within the sector
- Minimum 1 years’ experience as a Support Coordinator
- Knowledge and experience of the NDIS Support Coordination function and why it is important in building a Participant’s capacity and to the overall success of the NDIS
- Knowledge of relevant service providers across mainstream and other community service sectors, including mental health and disability providers
- Excellent written and oral communication skills
- Knowledge of MS Word, Excel and other online client reporting systems
- Excellent time management skills and demonstrated ability to work unsupervised
- Demonstrated ability to respond and adapt to competing priorities
- Intermediate understanding of Organisational Management systems
**Essential requirements**:
- Must have own vehicle
- Australian Driver’s Licence
- Current NDIS Worker Screening Check
- Current Working with Children Check
- NDIS Orientation Certificates
