Alya are a registered NDIS provider of disability support services specialising in mental health, psychosocial disorders, and complex physical and intellectual disability presentations. We are very proud to be one of only 15% of providers that are registered to provide support to NDIS participants. We deliver services across a wide geography in Southeast Queensland including Brisbane, Sunshine Coast and Gold Coast.
Our Case Managers (Participant Support Officers - PSOs) manage a caseload of NDIS participants, providing them with supports via effective deployment of our field-based Mentors (support workers.) Why do we call them Mentors? Not only do they support our participants with in-home care and community access, they also work with them to develop their life goals - empowering participants to live a meaningful, beautiful, developing life.
As a Case Manager (Participant Support Officer) at Alya you will foster and develop strong and effective relationships with a network of stakeholders. These include Alya PSOs, participants, their families, health professionals, occupational therapists to name a few. Your ability to build trust-based relationships with a complex ecosystem of stakeholders will be key to your success in this role.
As we continue to grow, we are looking for a capable, confident, and proactive Case Manager (Participant Support Officer) to work with us to provide best-in-class, innovative, support to our participants in the Sunshine Coast area.
If you have strong values-based beliefs that centre around integrity, care, and service and have a great ability to build trust-based relationships with those around you we would like to hear from you. You will have experience case management in the social and communities services sector, or similar. Perhaps you are a support worker with disability support experience looking for the next step in your career. Maybe you are a Support Coordinator looking for a support delivery centric role. This role is based out of a shared office at Caloundra for two days a week and working from home for the remaining hours/days. You will work with other Case Managers/PSOs and the corporate team to deliver timely and effective supports to Alya participants. You will be required to travel to meet with participants and other stakeholders.
