We have an exciting opportunity for a **part-time (0.8 FTE) Care Manager** to join our **Care Finder team in Abbotsford**.

**About the role**
This is a **part-time, maximum-term position until 30 June 2026** for an experienced and enthusiastic community care professional. You will provide independent, ongoing advice and guidance to clients across the **Eastern Melbourne region**.
Key responsibilities include:
- Delivering assertive outreach to identify, engage and build rapport with potential clients.
- Building networks with local intermediaries to strengthen client access to services.
- Taking a multi-dimensional approach to assessing client needs.
- Supporting clients to access My Aged Care packages and other relevant community services.
- Promoting Care Connect’s service model and value proposition.
- Providing customer service excellence to clients and their representatives.
**About you**
You are an engaging professional who values relationships and confidently delivers person-centred care plans. With a commitment to respect and confidentiality, you bring:
- Excellent interpersonal, communication, networking and negotiation skills.
- The ability to prioritise, delegate and organise tasks effectively.
- Experience managing client budgets and knowledge of Home Care Packages, Aged Care Standards, and community service networks for older Australians and their carers.

**What you’ll bring**
- Previous Care Management experience in Aged Care.
- Diploma or degree in Health/Allied Health, or 6+ years’ relevant experience.
- Strong communication, advocacy and interpersonal skills.
- Proven time management, prioritisation and budget management ability.
- Knowledge of upcoming Aged Care sector changes.
- Current Australian driver’s licence and reliable vehicle.
- Proficiency in Microsoft Office and willingness to learn client database systems.
