**About the Role - Care Manager - Home Care Packages**

We are seeking an experienced, enthusiastic community Aged Care professional to provide independent, ongoing advice and guidance to our community clients across **North/East Melbourne - VIC **. You provide case management advice and support that is respectful of, and responsive to the preferences, needs and values of individuals. You work collaboratively with Aged Care - Home Care Package clients, their carers and representatives to assess needs, provide consumer choice and develop sustainable, flexible, personalised and culturally appropriate care plans to meet their personalised goals.

In this mobile role you:

- Review intake information, engage new clients, establish the home care agreement, person-centred care plan, budget allocations and connect clients with local services and supports to meet their goals
- Network with Care Connect team members, stakeholders, and partnered service providers on behalf of clients and Care Connect
- Conduct annual reviews for all clients, revise care plans, goals and services to maximise budget utilisation
- Ensure interactions are conducted ethically, fairly and within the statutory legal and contractual requirements
- Attendance at hub office in Abbotsford, VIC for training and team meetings on occasion - up to twice monthly

**About You**

To be successful, you have:

- Completed tertiary qualification (degree or equivalent) in health or allied health discipline
- 4 years or more case management experience or equivalent
- Knowledge and demonstrated understanding of relevant legislation
- Sound knowledge of information technology
- Excellent written and verbal communication skills
- Proven experience and skills in the management of brokered funds
- Ability to maintain accurate and current care plans and documentation
- Ability to provide accurate and timely reports, as needed
- Work effectively in a team and autonomously from home
- Manage confidential information appropriately in accordance with best practice and Quality Standards
- Ability to work with set KPIs and achieve targets within agreed time frames
- Experience in the Microsoft Office suite and exposure and/or ability and confidence to acquire skills for in-house client database systems

Qualifications required for the role:

- Tertiary qualifications in allied health, social science, health/welfare discipline, or a related field
- In the absence of completed tertiary qualifications, experience in the delivery of person-centred practices in non-complex and/or complex case management and/or demonstrated experience with complex service-oriented offerings in Home Care or Aged Care services

- A valid Working with Children Check supplied prior to employment
- A statutory declaration confirming Australian residency status (an international police check/s may be required for non-residents)
