As a Wellness Coordinator at Gracewood you will empower residents to live life their way, supporting our residents by coordinating services that enhance their independence and wellbeing. Your comprehensive knowledge of services and experience within the industry, combined with tact, discretion and agility in your communication style will enable you to inform and empower residents to make the best decisions in relation to the services required for them to continue living their optimal lifestyle.

**Key Responsibilities**:

- Work in conjunction with our dedicated Village Care Facilitator who will assess and identify residents' support needs in accessing services
- Welfare checks supporting the support plans put in place.
- Initiate and promote awareness of Gracewood Community Wellness Forums including those from external providers.
- Assist residents in accessing services though our dedicated Care Facilitator.
- Foster residents' self-management and relationships.
- Ensure culturally appropriate service delivery.
- Maintain accurate records and documentation.
- Promote safe use of resources to reduce isolation and improve health.

**Key Requirements**:

- Significant experience in a related field within Home Services, Retirement Living, Aged Care or similar.
- Strong negotiation and communication skills.
- Team player with a good understanding of WHS practices.
- Computer literate with intermediate to advanced MS Office skills.
- Empathy, energy, patience, and flexibility.
- Willingness to embrace BaptistCare’s values and ongoing education.
