**Accountabilities**
- Pro\/ide, improve, maintain effective case management services under the Home Care Package Program (HCP)
- Develop, maintain and improve CHSP services
- Management
- Community Promotion

- Relationships of the Coordinator
- Team Supervision
- Oversee Programs - At all times remain up to date on information on Australian
Government Programmes, specifically HCP, CHSPand in contemporary service delivery methods.
- Perform any other task/s that are assigned that you are competent to perform and that can be done safely

**Job Requirements**
- Typically, 3 years case management and assessment experience in aged and /or community care.
- 3 5 years' experience in supervising and providing feedback to staff.
- Experience in consumer directed care, person centred support, case management and coordination
- Experience working with people with a disability including people wilh acquired brain injury, progressive neurological condition and/or complex disability.

**Knowledge**
- Knowledge and understanding of HCP, community services and supports for the ageing and disability health sectors.
- Detailed knowledge of the administration of Home Care Packages and the Commonwealth Home
Support Program.
- Knowledge to implement service delivery models that empower consumer/participants with flexibility, choice, capacity building and transparency
- Extensive knowledge of the Aged Care Quality Standards.
- Knowledge of issues involved in supporting and caring for older people and people with disabilities in the community and the aging process
- Understanding of financial impact of package budget and fees
- Contemporary knowledge of and understanding of the issues and needs of older people, their families and carers.
- Principles of professional boundaries.
