**Purpose of Role**
This purpose of this role is to coordinate disability support and care through case management as well as provide leadership, support and guidance to care staff ensuring client needs are met

**Main Duties & Responsibilities**
Core objectives include:

- Support clients to understand their NDIS plans, goals and objectives and use of the NDIS portal
- Assist clients to strengthen their capacity to establish networks, maintain support services, linkages and relationships
- Strive for solutions that proactively reduce client barriers, risk and maximise outcomes Implement strategies to ensure seamless communications across all networks
- Work with external service providers and suppliers to ensure maximum value for money is achieved
- Develop and maintain quality working relationships with each client and their stakeholders
- Participate in internal monitoring and audit processes to develop and maintain on-going best practice and ensure that the organisation maintains external accreditation as required
- Actively participate in staff and project development
- Carry out comprehensive and accurate client assessments
- Maintain appropriate records and documentation

- Fulfil other duties as required by management and other department personnel as requested/required_
- **Requirements**_
- Own vehicle
- First aid certificate
- NDIS Work Screening Check
- WWCC
- Minimum 1 year experience in the field or qualification equivalent

Preferred days of work - Monday, Wednesday and half day Friday

**Job Type**: Part-time

Pay: From $30.00 per hour

Expected hours: 20 per week

Work Location: In person
