**Description**:
The role

This role of the support worker is to provide exceptional service whilst supporting participants to meet their goals.

**This includes**:

- Provide appropriate levels of support to maintain and improve the clients level of independent functioning including participation in the community.
- Respond to the client's needs and collaborate positively with their families, support people and others in their lives.
- Keep accurate and complete records of activities via progress notes.
- Daily tasks will include transportation of clients to appointments/shopping, laundry, bed making, light housekeeping, limited personal care, social support, and companionship.

What do you need to thrive and succeed within this role?

To be successful in this role we are looking for someone who has Certificate III in Individual Support (or equivalent) or skills, experience, a passion to make a difference, and the following attributes:

- Be dedicated to empowering people with disabilities to live their best lives.
- Be a true people person who is passionate, patient, empathetic and resilient.
- Experience providing psycho-social support - Experience with complex behaviours would also be highly regarded.
- Regular access to a reliable vehicle to assist with client transport (some mileage reimbursed).
- Be reliable and have flexibility with shifts and rosters.
- Ability to work in partnership with individuals to achieve their goals.
- Able to work with limited supervision.
- Strong interpersonal skills and ability to communicate with a range of stakeholders.
- The capacity to meet the physical requirements as some manual handling tasks may be involved.
