An opportunity exists for an enthusiastic and experienced social worker to join our collaborative and dynamic Community Based Rehabilitation (CBR) team. CBR provides quality rehabilitation services within the division of Chronic and Complex Care.

This is a permanent role. Part time and/or full time available.

- Commitment and passion for providing exceptional patient centred care;
- To foster an environment where clients feel empowered to independently manage their lives;
- To demonstrate autonomous behaviour, independence of thought, awareness of own effectiveness and internalised responsibility.
- To demonstrate understanding of, and sensitivity to, ethical and multicultural issues.
- A minimum 3 years’ professional social work experience, preferable in a hospital/health care setting
- Highly developed interpersonal, written and electronic communication skills
