**About the role**

The Social Work team provides patients with additional support to improve psychological and social functioning and we ensure each patients social, emotional and cultural needs are met. We minimise the negative impacts of illness and hospitalisation on patients. We are a dynamic and innovative group who is always willing to share knowledge and peer support. The Social Work team is made up of 30 plus social work clinicians, providing opportunities for career progression. You will be supervised by an experienced senior social work clinician.

There are regular department in-services, journal clubs, external professional development opportunities, you are able to access study leave for higher education. There are many opportunities to develop your career and knowledge.

**Key Responsibilities**
- Ensure the smooth transition of clients through the health care system, liaising with relevant staff and community agencies to ensure continuity of care for individual patients / clients and their families
- Participate and fulfil requirements of allied health and discipline specific orientation, induction, mentorship, supervision and performance management programs
- Undertake comprehensive psychosocial assessments
- Support Social Work students and Garde 1 staff supervision
- Communicate with a wide range of professionals
- Complex discharge planning
- Collaborate in pursuit of patient and team needs/goals

**Skills and experience**
- Eligibility for practising membership of Australian Association Social Workers Membership (must have a bachelor or qualifying master’s degree)
- Demonstrated ability to monitor, evaluate and modify own performance through reflective practice
- Ability to adapt, be flexible and learn quickly
- Ability to plan and problem solve
- Demonstrate sound knowledge in Social Work theory and practice skills
- Working knowledge of subacute or community hospital systems and resources

Social Work positions are paid in accordance with the following agreement and classification:

- Allied Health Professionals (Victorian public sector) (single interest Employers) enterprise agreement 2021-2026. The classification is Social Work Grade 2,Year 1 - Year 4 (SC21 - SC24)which ranges from $42.18 - $48.49 per hour.
