This is an exciting time to be joining our growing team We have an exciting opportunity for an **experienced and driven NDIS Support Coordinator** to join our collaborative, dedicated team based in Liverpool.

- **About the role**:_

The primary purpose of this role is to work collaboratively with participants, their families, service providers and other stakeholders to ensure the implementation of individual NDIS Support plans and achievement of participant goals. You will be responsible for matching participants with community-based, funded and/or mainstream supports to fulfil their individual needs as per individual NDIS plans, whilst providing continuous support and assistance across the timeframe of each plan. You will support the participant to build capacity to achieve greater independence to self-direct their own services and supports.
- **About you**:_

Interested? This may be your next exciting new role if you have:

- A tertiary qualification in a relevant discipline or extensive relevant experience in working in disability services, social work or a similar environment.
- Experience and skills to work competently alongside people, their family and carers in their local community to build their capacity to participate in the community and manage some or all aspects of their plan.
- Experience working with people, their families and carers to identify and access opportunities for education and employment.
- Well-developed communication (both oral and written) skills with the capacity to prepare reports and correspondence in clear and concise language and the ability to assimilate information from other, present information in a manner appropriate to the purpose and audience.
- Demonstrated ability to identify, measure and report on outcomes.
- You have well-developed interpersonal skills with the capacity to liaise effectively with a wide range of customers and service providers.
- Ability to work autonomously and cooperatively as a member of a team.
- Knowledge of the NDIS Act 2013, Disability Act 2006 and other relevant legislation.
- Knowledge of the current trends in service delivery in the community sector, particularly the changing landscape of the disability sector.
- Knowledge of the NDIS Operational Guidelines.
- Experience working in an NDIS environment.
- Must have an NDIS Worker Screening Clearance, Working with Children Check Number and current First Aid Certificate.

**Salary**: $32.00 - $35.00 per hour
