We are seeking a compassionate and driven individual to join our team as a support coordinator.

**Role Purpose**:
As a Disability Support Coordinator, you will play a critical role in helping NDIS participants understand their NDIS plans, set up their supports, and strengthen their ability to manage their own lives. You will empower participants to explore opportunities, build support networks, and maximize the value of their plans.

**Primary Responsibilities**:

- Assist NDIS participants in implementing their plan and developing their capacity to self-manage.
- Help participants connect with their community, strengthen informal support networks, and explore alternative support options.
- Act as the main point of contact for participants, addressing service issues, complaints, major changes, and plan reviews.
- Support participants in preparing for their NDIS plan review, helping them assess progress towards goals and identify new ones.
- Assist with decision-making on life transitions, such as housing options.
- Facilitate participants’ access to mainstream, community, and informal supports, and assist in negotiating services, prices, and service agreements.
- Provide support in creating service bookings and managing budgets for different support categories.
- Liaise with plan managers and relevant stakeholders to ensure proper management and allocation of funds.
- Offer guidance to participants on resolving issues, managing service agreements, and understanding their rights and responsibilities.
- Maintain accurate records, complete invoicing, and ensure timely action on referrals.

- Proven conflict resolution and problem-solving abilities.
- Strong administrative skills, including time management and task coordination.
- High level of computer literacy, with confidence in using various systems and technologies.
- Ability to work autonomously while building and maintaining strong relationships with participants, families, and service providers.
- In-depth knowledge of the NDIS Price Guide, legislation, and rules surrounding "reasonable and necessary" supports.

**Essential Requirements**:

- Motor vehicle and valid driver’s license.
- Comprehensive car insurance.
- National Police Check.
- Working with Children’s Check (WWCC).
- NDIS Screening Check.
- PRODA Registration.
