**Your opportunity**:
You will work under the delegation of the social work team at Nambour Hospital, assisting with patient care including discharge planning, future care planning and practical supports. Tasks may include direct and indirect patient tasks. A key area for this role is supporting patient transition to residential aged care by maintaining relationships and completing records that contribute to patient satisfaction, team performance and reduced patient length of stay
Social work in the hospital can be a complex and challenging environment with regular disclosure of complex social issues such as drug and alcohol abuse, domestic and family violence, elder abuse, death and dying. You are encouraged to consider your aptitude for working in this environment prior to applying.
