As a Disability Support Worker, you will play a crucial role in providing personalised support to individuals with disabilities, with a focus on those with mental health needs (desirable). You will work collaboratively with our clients to promote independence, well-being, and community integration.

**Responsibilities**:

- Provide one-on-one support to individuals with disabilities.
- Assist clients in daily living activities, including personal care, meal preparation, and household tasks.
- Foster a positive and inclusive environment that promotes the social, emotional, and physical well-being of clients.
- Collaborate with a multidisciplinary team, including healthcare professionals and family members, to ensure comprehensive care and support.
- Monitor and report on the progress of clients, documenting any changes or concerns, with knowledge of Care Plans, Support Plan, Mental Health Plan and Behaviour Support Plans.

**Qualifications**:

- Previous experience as a Disability Support Worker.
- Relevant qualifications in disability support, or a related field.
- Strong understanding of the challenges faced by individuals with disabilities, particularly those with mental health needs.
- Excellent communication and interpersonal skills.
- Empathy, patience, and a genuine commitment to improving the quality of life for individuals with disabilities.
- Ability to work independently and as part of a team.
- Current First Aid and CPR certifications, Certificate IV in Disability/Mental Health, NDIS Screening Check, WWCC and Police Check
