**About the opportunity**:
Cosy home care has a tremendous opportunity for mental health support workers in Melbourne (specifically in Ivanhoe) to join our vibrant team and provide holistic care and support to the NDIS participant(s) within a purposeful residential facility.

This position forms part of our Individualized Services team that specialize in supporting people with psychosocial disability. The Support Worker position focuses on assisting participants to meet the goals within their individualized NDIS plan. The NDIS aims to provide people with choice and control over how, when and where those supports are provided.

Duties of the role will include:
**Duties & Responsibilities**:
This role will involve:

- Engage and develop rapport with participants who have a psychosocial disability;
- Establish good working relationships with NDIS participants and work effectively with them to define and achieve the goals as identified in their NDIS Plan;
- Enhancing the participant’s capacity to manage their health, welfare and life needs;
- Provide regular person-centered supports in social, community participation and activities of daily living.
- Engage and support family and/or careers in the recovery process, by linking them to internal team members who can facilitate linkages to services to meet their own individual needs.
- Work both autonomously in a community outreach environment, and collaboratively within a team to deliver high quality psychosocial supports.
- Assisting him/her with living with complex behavior, participate and engage in social and community activities.

To be eligible for this position, you **must**:

- have relevant qualifications in Mental Health or Allied Health Fields (diploma level or above)
- Minimum 2-3 years of experience in mental health or with clients with Autism
- Experience in a similar role with housing experience (highly desirable)
- be able to drive and own a functional working vehicle that you are happy to transport our clients to their appointment
- be able to support clients to meet their individual goals
- be able to develop skills and encourage independence & healthy relationships
- An understanding of the National Disability Insurance Scheme (NDIS)
- hold current Working With Children's Check
- hold current valid First Aid Certificate
- Own car and have full comprehensive roadworthy vehicle insurance
- Patient and emotionally resilient

We are offering a set roster. As a successful applicant, you will be assigned to a specific client and provided with a fixed number of shifts per week. The rosters will contain a combination of weekday and weekend shifts as well as flexible hours.
