**About the role**

Our** Intensive Family Support program** offers voluntary in home family support to families who are at risk of entering or re - entering the Child Protection System. Through case management and effective relationship building, we aim to strengthen families so children can live at home safely, free from abuse and neglect.

As a **First Nations Identified Case Manage**r (Child & Family Practitioner), **you will engage and provide intensive support to at risk families, holding a caseload comprised of identified families where possible,** to help them make positive choices in their parenting. Using your practice framework as well as Structured Decision Making tools, you will **undertake an initial assessment** of a family’s strengths and needs, and **develop case plans** that build on the family’s resilience to enable them to overcome risks and increase the family’s functioning.

Another key function of this role will be your contribution to community development, improving connection with services and outcomes for First Nations People.

**Duties**
- Provide holistic and effective support to Identified families
- Provide cultural consultation for Identified families throughout the case management continuum including:

- Review of all new referrals and provide advice/recommendations during allocation meeting
- Provide advice/recommendations directly to intake worker prior to allocation meeting if unavailable to attend.
- Attend cold calls for Identified families at intake stage
- Attend initial home visit for all Identified families allocated to non-Identified CFPs
- Participate in CAPs for Identified families as required
- Participate in key decision-making stages for Identified families as required
- Provide cultural consultations for Identified clients in adjacent programs as required.
- Attend relevant community network meetings, promote the Intensive Family Support Program within the community and relevant agencies, identify opportunities for increased community connection and support for Identified families.
- Create and maintain a calendar of special events for Indigenous families and collaborate with IFS team leaders to plan events
- Participate relevant working groups and in quarterly meetings with IFS leadership team to continue to review role, requirements and provide a cultural lenses for the service as a whole.

**Skills and Experience**
