**Location: Doncaster**

We’re looking for a compassionate and proactive **Disability Support Worker** to support a participant living with **Prader-Willi Syndrome**. This is a rewarding opportunity to help someone build independence, social awareness, and lead a fulfilling life.

About the Role:
You’ll be supporting a participant through a variety of meaningful activities and daily routines, including:

- Social outings to parks and community spaces
- Supporting gym visits and physical wellbeing
- Assisting with appointments, meal preparation, and domestic tasks
- Encouraging social capacity through shared activities and engagement
- Active overnight support
- Mealtime preperation and assistance

Key Responsibilities:

- Provide person-centred support aligned with NDIS goals
- Assist with daily living and domestic tasks
- Facilitate social and community participation
- Follow individual support plans and therapy strategies
- Maintain accurate progress notes and report incidents as needed
- Collaborate with families, team members, and allied health professionals

Qualifications & Requirements:

- Certificate III in Disability or equivalent (essential)
- NDIS Worker Screening Check (mandatory)
- Working With Children Check (mandatory)
- Experience supporting individuals with Intellectual Disability, ASD, ADHD, OCD is highly desirable
- Strong communication skills and a genuine passion for supporting others
