About the Role
The EWE program is a multi-professional approach to prevention of delirium and early restorative care in older patients.
- Contribute to supportive systems for older inpatients to reduce delirium and functional decline.
- Provide a defined range of screening assessments and treatments for patients as identified and delegated by the multi-disciplinary team, and as allowed by legislation and guidelines.
- Initiate changes to treatment programs under the direction of the multidisciplinary team using results from standardised assessment and treatment interventions.
- Provide advice and reinforce education on a defined range of topics to patients or groups of patients.
- Lead group treatment sessions together with a health professional as part of the Eat Walk Engage program as delegated by the multi-disciplinary team, including feedback to the referring health professional on client performance.
