**ROLE**:
We are looking for an experienced Support Coordinator to hit the ground running You will be based out of our Epping Office and take a portfolio of between 10-30 clients, requiring 20-30 hours of work each week.

In this role, the Social Worker/Support Coordinator will adopt a client-centred approach to promote client independence and improve social, health and wellbeing using service coordination and case management frameworks. Service provision includes (but is not limited to) comprehensive social work assessments, care planning, service coordination and client advocacy. The Support Coordinator will provide care to individuals and families in a range of community settings including private homes, public housing, supported residential services (SRS's) and other settings as required. They will also offer Psychosocial Recovery Coaching and direct the Support Work teams to build capacity and resilience of our clients living with psychosocial disability through strong and respectful relationships to live a full and contributing life.

**RESPONSIBILITIES**:

- To provide, support coordination, in line with individuals NDIS plan and acquire the support they require to manage their ill-health
- Provide clear and concise support to clients in line with the goals articulated in their NDIS plan
- Provide support coordination with a recovery-based focus
- Working knowledge of the available services and supports accessible to clients including (but not limited to) health, housing, employment, family, financial supports, physical health care and rehabilitation programs
- Maintain ongoing support and monitoring of the funds in the client’s NDIS plan.
- Work collaboratively with clients, their families and next of kin (including carers & children)
- Work collaboratively with internal and external service providers to facilitate services with regards to the client’s needs and goals
- Contribute to and participate in the planning, implementation and evaluation of the individual’s programs
- Adhere to all relevant current internal and external Policy and Practice, standards of discipline and specific practice standards and codes of ethics, including Department of Health guidelines
- Contribute to the development and maintenance of a supportive environment within the team.
- Achievement of billable hours targets
- Ensure that work and services are provided in a safe manner at all times by regularly reviewing practices and environment and by participating in OH&S training where required.
- NDIS systems: Participate in all elements of the NDIS system, including quoting, service agreements, managing individual NDIS budgets, NDIS reporting, plan reviews and invoicing
- Maintain accurate program records and statistics and comply with all administrative quality and compliance requirements and tasks
- Knowledge and experience with case noting systems and the ability to maintain clear and concise case notes. Experience using data platform Brevity (desirable)

**ABOUT YOU**:

- Willingness to enthusiastically undertake all relevant training, professional development, and up-skilling opportunities in accordance with industry and legislative requirements, and in order to provide support of the highest quality.
- The ability to remain flexible and adaptable to changing conditions while maintaining a high standard of professionalism; capable of working calmly under pressure.
- Proven time management and organizational skills.

**QUALIFICATIONS**:
A minimum of Certificate IV in Mental Health or other relevant tertiary disciplines is a pre-requisite of this role.

Most advantageous would be a Bachelor's Degree in Social Work, Human Sciences, or Science (Psychology). Pay rates will depend on area of study and experience.
