**About the role**

We have an exciting opportunity in this newly created role for an Exercise Physiologist with community experience, to join our multidisciplinary Aged Care Community and Home Support Team, to work with our clients in the Perth Metro Region.

This role will report to the Senior Physiotherapist, and you’ll be responsible for managing your own caseload of clients to assess their needs and plan individual and group exercise programs aimed at meeting their goals and preferences. The role will inspire and motivate clients to increase their quality of life by optimising focus on exercises to foster independent living and wellbeing. Working alongside a supportive team of Occupational Therapists, Physiotherapists and Allied Health Assistants to work towards goal driven outcomes.

**About you**

You’ll be self-motivated, have great communication skills with a wide range of people and bring drive and energy to your work. Your key experience will include:

- Bachelor’s Qualification in Exercise Physiology or a related field
- Accreditation with ESSA (Exercise & Sports Science Australia)
- Demonstrated solution focus and communication skills
- Strong administration and case documentation skills
- The ability to engage clients, care givers and care teams
- Demonstrated ability to identify needs, develop case plans and influence outcomes
- Demonstrated ability to actively collaborate in multidisciplinary teams
- National Police Clearance (obtained within the last 6 months)
