**Description**:
We are seeking a special individual to fill the newly created role of Client Journey Coordinator to join our Care Safety & Quality Team. This role is suited to a clinically qualified professional (nursing or allied health) who is passionate about supporting older people and guiding our teams across Aged Care services.
About the Role
**In this role, you will**:

- Support clients as they transition across the full aged care pathway, from community programs to residential and retirement living.
- Provide clinically informed advice to ensure care is appropriate, timely, and person-centred.
- Oversee onboarding processes, manage referrals, and contribute to client retention strategies.
- Collaborate with internal teams, hospitals, and My Aged Care assessors to ensure smooth and supportive transitions.
- Champion a customer-first approach, gathering feedback and driving continuous improvement.

**Skills and Experiences**:
What We’re Looking For
- Tertiary qualification in Nursing, Allied Health, or similar, with current AHPRA registration.
- Experience working with aged care programs (e.g. CHSP, restorative care, home care packages).
- Strong knowledge of My Aged Care systems and aged care funding pathways.
- Excellent communication and relationship-building skills.
- Experience in program coordination, client engagement, and data-informed improvement.
- Knowledge of retirement and assisted living models (desirable).
