Our Client is a leading health and family - based service providing a holistic approach to the wellbeing and needs of the Aboriginal and Torres Strait Islander community in Brisbane and surrounding areas.

**The Role**

**The role is a permanent, full time position - with the option for a 9 day fortnight.**

The Team Leader is responsible for leading the intake and triaging function of the program, coaching, supervision, case audits and supporting the team with complex cases, which includes going on home visits - In-field coaching of front-line staff to hone their practice and interventions skills when working with young people and families

The Team Leader supports a small team of Case Managers working across two Youth Programs:

- **Brisbane Youth Service**: Leading the coordination of tailored case management services and interventions for young people and their adult family members at risk of disconnecting with their family/ community, disengagement from education, training and employment, homelessness, or at risk of harm. Services are delivered as a whole-of-family approach, involving case management and programs.
- **Youth Justice Family Led Decision Making**:Leading the delivery of a small youth justice program, aimed at reducing recidivism in the youth justice system through ‘Family Led Decision Making’ and case managed supports to young people and family.

The goals of the programs are:

- connect to positive family support
- get a job or engage in training or education
- lead a healthy and violence-free life
- have a safe, stable place to live

**To Be Successful**
- **Bachelor Degree** in Social Work, Human Services, Social Science or similar, **with** **4 years’ relevant experience** in family support, child protection, youth support or similar

**OR**
- **Diploma** in Community Services, Social Work, Human Services, Child and Family, Youth Work or similar, **with 5 years’ relevant experience** in family support, child protection, youth support or similar
- **3 years Case Management experience**:

- **2 years experience in a Senior Worker / Team Leader role**:

- Demonstrated ability and comfortableness working in a performance driven environment to meet individual weekly, monthly and quarterly performance targets
- Demonstrated ability to build client agency, self-efficacy and facilitate change through client empowerment, while actively discouraging client dependence on supports
- Demonstrated knowledge and ability to provide behaviour change interventions that build client self-awareness and active change in achieving their identified goals
- Demonstrated ability to work with the whole-of-family, irrespective of primary client, in meeting the totality of family needs
- Demonstrated ability in coordinating multiple services in providing access to holistic care for clients, particularly integrating internal agency supports across multiple teams
- Demonstrated knowledge and understanding of the factors contributing to vulnerability, disengagement, and intergenerational trauma of client group
- Experience with case management for high risk and/or crisis clients, with high-level planning and organisational skills
- Knowledge of the supports available to target clients in the areas of mental health, substance misuse, education, employment and housing
