**ABOUT THE ROLE**
Fresh Start is actively seeking a passionate, enthusiastic, and purpose-driven team player willing to make a meaningful difference in the lives of others on their journey to recovery. As a full-time Case coordinator, this role will primarily be reporting to and working closely with the Residential Rehabilitation Manager.
You will play a pivotal role in the case coordination of the recovery program, ensuring high-quality residential care is provided and empowering individuals to overcome the challenges of addictions.
**The key responsibilities include**:
- Conduct the intake and assessment process, onboarding new clients to the program.
- Collaborate closely with Fresh Start’s team members across all the facilities (Housing and Residential managers and Allied Health teams) to ensure seamless program delivery.
- Keep a vigilant eye on the client's physical, emotional, and spiritual well-being, recording and addressing concerns promptly.
- Support the Case Management team and stay updated with each client's case records within a case management module.
- Actively contribute and facilitate in the client’s end-to-end recovery journey.
- Work closely with the Residential Rehabilitation Manager in areas such as recruitment, induction, and staff rostering.
- Respond empathetically to clients and families, answering any queries and addressing concerns.
- Willingness to engage in diverse organisational initiatives to foster growth and development.

**ABOUT YOU**
**Ideal Skills**
- **Have**:_
- Tertiary educational qualification in community services or other relevant areas.
- Similar experience working either in Mental Health or Recovery Centres.
- An understanding and non-judgemental attitude towards people with drug use and mental health problems, including those from diverse cultural backgrounds and the Aboriginal community.
- Well-developed interpersonal communication skills, flexible, and a committed team player.
- Excellent verbal and written communication skills.
- Demonstrated problem-solving skills and an ability to respond appropriately to emergencies.
- Respect for confidentiality.
- Driver’s license.
- Willingness to support Fresh Start’s Christian approach and programmes.

- Communicate efficiently with other people within the organisation and community.
- Work both independently and as a part of a team while being proactive and showing initiative.
- Portray positive behaviours, emotional maturity, positive coping skills and resilience.
