**About the Role**:
We are currently seeking a passionate and experienced **NDIS Support Coordinator** to join our team. In this role, you will assist participants to understand and implement their NDIS plans, build their capacity to manage supports, and connect them with appropriate services and resources in the community.

**Key Responsibilities**:

- Assist participants in understanding their NDIS plan and how to implement it effectively
- Coordinate and connect participants with appropriate services and supports
- Monitor progress and outcomes, and make adjustments as required
- Support participants during plan reviews and reassessments
- Maintain accurate documentation and case notes
- Work collaboratively with participants, families, carers, and service providers

**Requirements**:

- Previous experience in NDIS support coordination or a similar role
- Knowledge of the NDIS framework and person-centred practice
- Strong communication, problem-solving, and organisational skills
- Ability to work independently and manage a caseload
- Relevant qualifications in community services, social work, or allied health (preferred)
- Current driver's licence and reliable vehicle
- NDIS Worker Screening Check, Working with Children Check, and Police Check
- Knowledge about SIL and SDA is preferred

Pay: $30.00 - $40.00 per hour

Expected hours: 10 - 15 per week

Schedule:

- Afternoon shift
- Day shift
- Morning shift
- Rotating roster
