**Key Responsibilities**:

- Provide therapeutic interventions to address clients' physical, cognitive, emotional, and social needs.
- Collaborate with multidisciplinary teams to ensure integrated and holistic care delivery.
- Maintain accurate documentation and records of client progress and outcomes.
**Requirements**:
- Relevant qualifications and registration with the appropriate regulatory body for each profession.
- Previous experience in a similar role preferred but not essential.
- Strong interpersonal and communication skills.
- Ability to work effectively in a collaborative team environment.
