Located at The Royal Melbourne Hospital - Royal Park Campus**
**Your new team**
- A dynamic, diverse multidisciplinary team who strive to make life better for the people we work with, their families and carers
- We work primarily with adults aged 65, in an acute phase a mental illness, who need support in their recovery through an outreach model of care and with a recovery centred focus.
- A collaborative and respectful working environment where clinicians are encouraged to share clinical judgements and participate actively in treatment planning with the medical staff
- You will be well supported by the team and the wider nursing program
**Your new role**
- Provide care as directed by the Manager and Consultant Psychiatrist to the consumers under the care of the ICT team
- Use your exemplary communication skills to support and care for consumers at home, as well as communicating with families and other members of ICT and related services to ensure the best care is provided
- Be responsible for assessing, monitoring, implementing plans, documenting and reporting all care and interventions provided and a status update about the consumer to the team daily
- May be required to supervise medications.
- This is a multidisciplinary role and is open to Occupational Therapists, Social Workers and Psychologists._

- Ability to deliver evidence based treatment and interventions that support older people with complex mental health and/or ageing related needs, and their families and carers
- Participate in clinical decision making
- Capable to accurately prepare documentation in a timely manner
- Excellent communication skills and ability to engage effectively with consumers, families/carers, colleagues and other service providers
