**NDIS Support Coordinator**

**About the Role**

Disacare Solution is seeking a dedicated **Support Coordinator** to assist a diverse range of NDIS participants with unique needs and goals. Joining the Disacare Solution team means embarking on a rewarding journey, supporting individuals and families in navigating and achieving their NDIS plans and aspirations.

As a skilled Support Coordinator, you will connect participants with appropriate NDIS plans and services, facilitating access to essential supports. You will manage varying levels of coordination and transitions for individuals up to 65 years old, addressing a wide range of complex needs. Your role will also involve helping participants engage with community and mainstream programs to achieve their personal goals.

We are seeking someone with in-depth knowledge of NDIS regulations and reporting requirements, strong service delivery skills, and excellent time management abilities. You should be able to thrive both independently and within a team, especially under pressure. Most importantly, you should have a compassionate approach that prioritizes the well-being of individuals and their families. Building strong relationships with stakeholders, healthcare providers, participants, and their families is key to success in this role.

**Key Requirements**
- A degree or qualification in Social Work, Human Services, or a related discipline.
- A minimum of 1 year experience in Support Coordination for individuals with complex needs.
- Demonstrated understanding of NDIS regulations and prior experience in the NDIS sector.
- Strong reporting skills, including preparing detailed reports in compliance with NDIS requirements.
- Proven ability to meet KPIs, multitask, and manage time effectively.
- Experience working with culturally and linguistically diverse individuals.
- Previous Support Coordination or Case Management experience is essential.

**Key Responsibilities**
- Conduct initial assessments for new participants.
- Coordinate with service providers to meet client objectives and needs.
- Liaise with government agencies, community partners, and allied health professionals.
- Obtain quotes for services, manage service agreements, and monitor NDIS plan usage.
- Prepare regular Support Coordination reports for submission to the NDIA.
- Organize and present detailed information about participants’ support needs, capacity, and goals for NDIA review.
- Monitor participant plan outcomes and budgets, ensuring compliance and progress.
- Handle multiple participant cases independently, maintaining quality of care.
- Refer participants to allied health services with a focus on recovery.

**Job Type**: Full-Time
**Pay**: $ 85,000 Per annum

Pay: $85,000.00 per year
