**What You Will Be Doing**
The Transition Care Program is seeking an experienced, innovative and flexible AHA to work as a member of the TCP team.
The program is a jointly funded program (DHS/Commonwealth) and it aims to assist frail aged patients reach their maximum functioning capacity and to help determine their long term care arrangements.
The successful applicant will need to be flexible in terms of working across both residential and community areas. The AHA will need to demonstrate sound skills, and to have an understanding of the issues affecting patients with complex functional and cognitive needs and an interest in aged care issues.
The successful applicant will need to be highly motivated, and enthusiastic and to be able to work both independently and as part of a team. Further to this, the applicant will need to support allied health service provision and clinicians within the TCP program as required.
**What You Need**
We anticipate you will have:
- Allied Health Assistant Certificate III/IV or equivalent
- Experience in health care setting
- Well-developed interpersonal and communication skills
- Ability to meet physical requirements of the position
- Constructively communicates concerns and issues
- Capacity to work independently and autonomously
- Actively supports changes by adjusting work practices.
- Experience in working within subacute/Aged Care Allied Health settings.

Essential: Do you have the ability to plan and manage own workload?
Essential: Do you have a high standard of written and verbal skills?
