**The Position**:
We are seeking a highly motivated and enthusiastic Care Manager. You will provide care management activities and ensure the safety, wellbeing and positive outcomes for clients receiving a government-funded Home Care Package. Opportunities also exist to maintain your clinical skills through support and delivery of services to our client base.

You will be responsible for:

- Supporting clients to maintain their optimal level of independence and safety at home.
- Providing ongoing client assessment and monitoring to ensure care and services meet their individual needs, preferences and goals.
- Developing and maintaining client individualised budgets.
- Providing clinical support and expertise as required.
- Development and review of care and service plans in consultation with clients and their chosen representatives.

The position has a level of professional accountability and responsibility in line with SereneCare policies and procedures and legislative requirements of the Home Care Package Program. You will play an integral role in the SereneCare team, providing personalised services and clinical support with a high level of compassion, loyalty, and commitment.

***Key Selection Criteria**:

- Tertiary qualification in Nursing, Allied Health, Social Work or related field.
- In the absence of tertiary qualifications, demonstrated experience in care management and knowledge of Home Care Packages.
- Demonstrated experience completing client assessments and care and service planning within a community setting.
