**About the role**

Working across Northern NSW Local Health District (NNSWLHD) to facilitate a collaborative approach to providing person centred care for patients transitioning from hospital to discharge. Facilitating and enhancing communication between the patients, families, carers, clinical staff, out of hospital care services and primary care to improve the discharge experience for patients, carers, and families. Providing high quality clinical services and leadership as an integral part of the multidisciplinary health care team for patients/clients of the NNSWLHD.
