About the role

Tweed Community & Allied Health have an exciting career opportunity for a Clinical Nurse Consultant to provide clinical leadership and facilitating a coordinated, high quality palliative care service to the local community, with a case coordination model that involves generalist community nurses in providing direct clinical care. Providing a multidisciplinary approach, with an end goal of improving quality of life for those patients and their carers.

Working with members of the palliative care nursing team across the inpatient and community settings, providing a consultative and liaison focussed service aimed at supporting GPs, Community Nurses and the Residential Aged Care Facilities in the assessment, care and management of patients with life limiting illnesses.
