- Baulkham Hills office base with travel to carers homes
- Foster Care/Out of Home Care experience preferred

**The role**

We are looking for a passionate Case Manager to guide the Children, Young People and families we support. We work collaboratively to deliver services through a strength-based and person-centred approach.

**About you**

As an experienced Case Manager, you will:

- Manage a caseload of children and young people/adults and their families and carers
- Complete comprehensive assessments to ensure the development, implementation and review of individual case plans
- Facilitate access to services and provide feedback on their effectiveness
- Prepare reports and correspondence relating to the implementation and progress of case plans
