**About the role**
This position is within the dynamic, multidisciplinary Community Rehabilitation Team and involves the provision of _clinical case management services to complex clients including those with an acquired brain injury and complex neurological conditions._
**Key Responsibilities**
- Comprehensive assessments and plans
- Assisting clients to develop and met their rehabilitation goals
- Coordination of services and referrals for the complex client
**Skills and experience**
- This position would suit a Social Worker or Occupational Therapist who has knowledge of neurological conditions and the impacts on the rehabilitation process, and who is looking to develop an additional skills in supporting clients through the rehab process.
