We’re looking for someone with knowledge, practical experience and skills, to support participants and their families with a disability or allied health background to manage a caseload of participants with complex injury related needs as they transition back into their community following serious injury, including brain and spinal injuries.
**The opportunity**:
The purpose of the role is to ensure the best outcomes for our participants by navigating and facilitating the access, management and cohesion of services and supports following discharge from hospital and during their time of adjusting and settling into community life following serious injury. It will involve establishing, coordinating, and monitoring multi-disciplinary community-based rehabilitation programs with a focus on maximising the participants recovery and independence.
It’s an exciting time at icare, where no two days are the same; as a **Rehabilitation Case Manager** in the Lifetime Care Northern Region team you will:
- Guide participants and their families in setting expectations, understand what they can reasonably expect from icare and to foster a life-long relationship that is built on trust.
- Collaborate with internal specialist services and use of resources when establishing and coordinating requests for services and supports.
- Use a person-centred approach, support the participant in the early injury phase to develop a My Plan and assess and actively monitor the effectiveness of selected strategies through the My Plan process.
- Approve recommendations and funding regarding reasonable and necessary treatment, care and support within our decision-making framework and agreed functional delegations.
- Support participants and their families to resolve complex and urgent issues as they occur.
- Contribute to the knowledge management of research and advice on injury management and service delivery to people with severe injuries to identify gaps and barriers in service delivery and share this broadly across the business in collaboration with internal specialist services.
- Keep abreast of and understand the legislative framework and Guidelines and how these contribute and impact our decision-making framework.
- Engage immediately in the event of a serious incident or risk to the participant’s safety, assess and implement a plan to ensure the necessary supports are in place to provide the safety and wellbeing of the participant’s wellbeing.
- Transition participants to a less intensive support resource when the participant no longer requires a rehabilitation case management service.
**Your story so far**:
- An allied health or rehabilitation background (or a relevant degree in a health-related discipline or rehabilitation advisor), you are equipped with transferable skills to coordinate the rehabilitation services to people with disabilities similar to those supported by our schemes.
- Community Based Rehabilitation Case Management experience with working with people with traumatic brain injury and/or spinal cord injury would be required.
- Highly developed understanding of the needs and perspectives of people with disabilities.
- You’ll use your critical reasoning skills to ensure a high degree of integrity in the decision-making process, have excellent communication skills to build rapport and truly connect with people to build trust and familiarity with participants, their families and their rehabilitation team.
- Flexibility and insightfulness and an ability to tailor established service delivery to meet the circumstances of each participant.
- Ability to set clear expectations and engage in difficult conversations in a simple and empathetic manner.
- A full driver’s license and ability to travel regularly within Northern New South Wales, Newcastle and Hunter Valley regions.
