Sydney, Australia Chosen Therapies Full time

Our mobile Allied Health company Chosen Therapies is looking for a Social Worker to do some mobile subcontractor work across Sydney. Commencing with work in the Sutherland and St George Region but may progress to referrals over South West, Inner West and North Sydney depending on your willingness to travel. As this is a relatively new business referrals will grow as the role progresses.

The work involves providing high quality Social work assessments on elderly clients in their homes on Short Term Restorative Care Programs and Homecare packages. Most work involves liaising with patients and their families to help connect them with aged care services or patients with helping patients with complex care needs/social arrangements.

You will earn between $100-$150 + super for each thorough visit. Initial visits would be in person but follow up visits for some clients may be possible over the phone.

This would suit someone looking for a bit of extra work here and there but may progress to more regular work as the work load increases.

**Salary**: $100.00 - $150.00 per hour
