**About The Role**
Monash Health is the largest integrated health service in Melbourne and has world renowned research and teaching facilities. The Cognitive, Dementia and Memory service sits within Monash Health Community. This role is to provide care and management of clients of the Cognitive, Dementia and Memory Service (CDAMS). It will focus on innovative models of service delivery that are focused on best practice and patient centred care and will work collaboratively with management to support change and service improvement initiatives.
**About You**
You lead by example, thrive working in a dynamic, fast paced environment and are truly passionate about delivering a high standard of evidence-based care to all patients. You are an experienced clinician with experience in working within a multi-disciplinary team. You are an advocate of Monash Health’s Strategic Guiding Principles and are excited to be accountable for managing portfolios in our relentless pursuit of excellence, whilst possessing excellent written and verbal communication skills.
**What You Need**:
- Bachelor of relevant Allied Health or Nursing equivalent degree
- _ Demonstrated experience in a multidisciplinary team with at least 5 years post qualifying experience_
- Completion of a post graduate qualification in dementia care or working towards
- Australian working rights
