**About You**

We are currently looking for exceptionally passionate, professional, dedicated and experienced Disability Support Workers to welcome to our TLC team. You have embraced our motto **From Caring Comes Courage**.

You have the ability to work up to 38 hours weekly including weekends. Due to the support required for our participants, you must be available and willing to work a varied pattern of shifts across a 24/7 roster (including short shifts, night shifts, weekends and public holidays) to be considered for these positions.

**Essential Qualifications & experience**
- Current First Aid and CPR
- COVID-19 vaccinations - including booster
- Medication Certificate
- Manual Handling certificate
- National Police check - gained within the last 3 months.
- NDIS Worker Screening check
- NDIS Worker Orientation Module Certificates
- COVID-19 Infection Control Training
- Unrestricted access to a vehicle
- Full drivers licence

**Examples of some tasks & responsibilities**

**Position purpose**
- **As a Team Leader, you will provide support to our participants in their homes and the community to increase their individual skills, help build their capacity and achieve their goals. You will promote and provide environments that support the safety, independence, growth, health, and well-being of our participants. You will be the first point of contact for your allocated group of Disability Support Workers. You will provide leadership and support for your team to provide the best services they can to our participants.**_

**Main tasks**

Core objectives include the main tasks of a Disability Support worker role and additionally,
- Monitoring and supporting your allocated team of Disability Support Workers within their roles.
- Developing and encouraging professional growth within your team of support worker
- Allocating and defining tasks for the team to perform
- Work under general guidance from the senior staff at TLC and the participant seeking support, and their family, within clearly defined guidelines.
- Support participants in age-appropriate activities and tasks, in SIL, Respite, in home support and community access.
- Support participants to access activities and outings in the community.
- Provide direct care assistance to each participant in your allocated group in accordance with their service plan and as directed by the Service Manager.
- Work in accordance with the medication administration policy, work health and safety procedures and participants rights policies including privacy and confidentiality.
- Monitor the safety and well-being of the participants and report any concerns to the Service Manager.
- Provide 1:1 or small group training and support in response to individual support worker needs.
- Ensure all members of your team are completing the appropriate documentation as delegated from senior staff. Documentation including GOM reporting, incident and injury reports, any recording charts, risk assessments, medication charts, and other documents that are associated with operations of TLC
- Completion of medication audits
- Relevant information on high quality support is provided to your team to encourage on-going quality improvement.
- Check that all participants notes are being completed and to the standard set by senior staff. Contact, educate and support if more information or details are required
- Participate in planning and working with participants, family/carers, case managers and other professionals to support the participants individual goals.
- Update and track any changes to goals from the NDIS Plan
- Take a proactive approach in each of your participants supports and needs.
- Participate in Performance Development to identify ongoing training and support opportunities for your team of support worker
- Work with rostering to maintain the roster to ensure each participants supports are being met.
